AFC RN Case Manager
Minimum Requirements - Education and Experience: The A.F.C. Nurse will be an R.N. with experience working with the disabled/ frail elder population.
Care Manager
Minimum Requirements - Education and Experience: A Bachelor’s Degree, a social worker license from the Massachusetts Board of Registration in Social Work, and at least two years’ recent experience working with elderly or disabled adults; or A Bachelor’s Degree and two years’ clinical experience in the care of elderly or disabled person.
